Each participant undergoes a comprehensive assessment by a multidisciplinary team including doctors, psychologists, social workers and therapists. The assessment examines the physical, mental, emotional and social condition and identifies the main needs and goals.
Based on the assessment, we build a personal treatment plan that combines multiple initiatives. The plan includes clear goals, timelines and milestones for tracking progress. Each plan is updated regularly according to the participant's progress.
Each participant receives ongoing personal guidance from a dedicated case manager. The case manager coordinates between the different initiatives, ensures that the participant receives all necessary services and provides emotional and practical support throughout the entire process.
We conduct periodic evaluations to track progress and adjust the plan as needed. The evaluations include objective measures such as functional improvement and subjective measures such as satisfaction and sense of well-being.
